1. Filled out and signed by an applicant - relativesApplication for the certificate for the transfer of human remains to Bosnia and Herzegovina“, (Justice of the Peace or Notary Public must be a witness of signature) - original.

  2. Depending on the burial area in B&H (Federation of BiH, or Republika Srpska, or Brcko District of BiH ) filled out and signed by an applicant - relatives (Justice of the Peace or Notary Public must be a witness of signature) - original

    a) „Request for the transport of the human remains from abroad to the Federation of B&H“; OR

    b) „Request for the transport of the human remains from abroad to Republika Srpska“; OR

    c) „Request for the transport of the human remains from abroad to the Brcko District of B&H“; 

  3. Original Death Certificate (It must be issued with an Apostille stamp).

  4. Original official translation of the Death Certificate into one of the official languages of Bosnia and Herzegovina (Bosnian, Croatian, Serbian). Web page of Official translators: www.naati.com.au).

  5. Embalming Certificate (certified copy).

  6. Non Contagious Disease Certificate - Medical Certificate of Death (certified copy).

  7. Human Services Department Certificate of No Objection for the Body Shipment out of Australia (certified copy).

  8. Funeral Directions Declaration on the Contents of the Coffin (the coffin must be prepared for shipment according to international standards – hermetically sealed, and proof that the body was not previously buried) - certified copy.

  9. Letter of Acceptance - Burial Place Reservation for the remains of the deceased from Funeral home or religious institution from Bosnia & Herzegovina and the name of the consignee.

  10. Certified copy of Valid Bosnia and Herzegovina Passport (if the deceased did not have a valid BiH passport, other evidence of citizenship of Bosnia and Herzegovina, such as CIPS ID card or Citizenship Certificate (date of issue not older than 6 months) or other evidence of BiH citizenship document (date of issue not older than 6 months) - if there is no photograph on the document additional identification document with a photograph is required (Australian or other Passport, Personal ID, Driver Licence).

  11. Money Order (for Consignment Note 47 AUD, plus express postage 8 AUD) - total 55 AUD payable to the Embassy of Bosnia and Herzegovina.

    NOTE: - personal cheques are not accepted

                        - all above documents will remain at the Embassy

 

If the deceased was a citizen of Bosnia and Herzegovina, the death was not caused by a contagious disease, and the body has not been buried, consignment note may be issued without approval from authorized state institutions from Bosnia and Herzegovina.
